The only way to get loot is to kill the rift guardian who drops a lot of high quality loot. However, in GREATER RIFTS, you are on a time limit so NOTHING drops loot. You can also earn the Death’s Breath currency from killing elites. You can get legendaries from opening basic chests and breaking weapon racks. Everything can drop high quality loot too. In a standard rift everything drops loot. These allow you to play at difficulties beyond the standard T16 cap. If you select Greater Rift then you will select the difficulty of the greater rift when you start it. If you select Nephalem rift, the difficulty is based on whatever difficulty you picked to start your game.
